dc.description.abstract This study presents the design and development of a web-based information system tailored to support Community Health Workers (CHWs) in Wakiso District, Uganda. CHWs are instrumental in delivering primary healthcare services at the community level, yet their effectiveness is constrained by manual data management practices characterized by inefficiencies, inaccuracy, and limited access to timely health information. Employing a Design Science Research methodology, the project involved stakeholder consultations, iterative system development, and usability testing to ensure contextual alignment. The resulting system, built using PHP, MySQL, HTML, and CSS, features role-based access, offline functionality, real-time data entry, and automated reporting. It enables CHWs to log visits, update patient records, submit referrals, and receive supervisory feedback; significantly improving service coordination and data reliability. Testing confirmed that the system met key user requirements, including operability in low-connectivity environments and ease of use. The findings affirm that user-centered digital tools can enhance health information management in resource-constrained settings when grounded in local needs and technical realities. The study recommends further training, hardware support, and policy endorsement to ensure sustainability and scale. This work contributes to the growing body of evidence advocating for localized, low-cost digital health innovations to strengthen community-level healthcare systems. en_US
