dc.description.abstractThe aim of the study was to examine the current methods of records documentation and developing a training manual for the documentation of health records for Ndejje Health Center IV. The objectives of the study were to; analyze the types of health records that are eligible for documentation at Ndejje Health Center IV, examine the methods of records documentation used at Ndejje Health Center IV, find out challenges that are faced by documentation of health records at Ndejje Health Center IV, developing a training manual for the documentation of health records for Ndejje Health Center IV. The study used a case study research design while adopting qualitative approaches for data collection. Data was obtained through the use of questionnaires, interviews and observation methods. In the study, records officers, doctors and nurses were purposively selected to participate in the study. The study established that NHCIV documents a number of records which include; Patient Case notes, X-rays, Patient Registers, Nursing and Ward Records, Progress Notes and Laboratory reports. There were various methods of documenting health records at NHCIV which include; Charting by exception, Narrative method and The nursing process. The benefits of documenting health records include; makes the continuity of care easier. promotes better communication and dissemination of information between health care providers, address complaints or legal processes, supports clinical audit, research, allocation of resources and performance planning, supports effective clinical judgment and shows how decisions were made relating to the patient's care. The study also revealed different challenges faced during health records documentation at NHCIV which include; An absence of clarity, inaccuracies, spelling mistakes, missing information and limited skilled personnel in documentation of health records. The study established the recommendations for improving health records documentation at NHCIV and these included; increase in the funds, training of staff on proper documentation of health records, automation of health records documentation process and adaptation of a health records documentation training manualen_US
