dc.description.abstractRecord is a document regardless of form or medium created, received, maintained and used by an organization (public or private) or an individual in pursuance of legal obligations or in the transactions of business, of which it forms a part or provides evidence Medical records are records that consist of a patient‘s demographic information like name, address, age and gender as well as that patient‘s medical history, test results, vaccinations, and other medical histories Records center A building or part of a building designed or adapted for the low-cost storage, maintenance and communication of semi-current records pending their ultimate disposal Records management has been defined as the systematic organization of all recorded information whether it be on paper, tape, disc, film, or any other medium from its creation, through its use, to its storage and the final disposition, either in permanent archival storage or in its legal destruction. What this means is that the recorded information of an organization must be managed throughout its life cycle, from the moment it is created, throughout all the years that it might be kept, to the day it is destroyed.
