    Abstract
Handling patients’ medical information in hospitals and health centers today requires frequent supervision and competent administration when handling patients’ physical examinations and medication. Issuing medical reports from doctors with patients’ medical information and billing patients serve as key factors for the flow of business transactions at Mirembe medical Center, Najjanankumbi and Malcom clinic, Kisasi. Unfortunately the current patient medical information management system makes diagnosis of nomadic patients difficult for the patients have none of their medical information with them. The patients’ medical information is kept at the clinics and the patients have no quick access to them especially if they change clinics or hospitals. This research project is aimed at easing diagnosing nomadic patients by doctors or health personnel’s. In order to achieve this goal, investigations, observations and case studies were carried out and data was collected and analyzed about the current situation. Access to medical records has been eased since the patients or their next of kin can write the medical information using the app or take a photo of the medical information so that it can be reviewed by any other doctor hence easing diagnosis of nomadic patients. The methods used to develop the application include iterative waterfall model approach, prototyping, data flow diagrams, logical and entity relationship diagram were used to come up with the design of the application. We used the following languages; C sharp, created a MySQL database and php with json for connecting to the database, posting to and retrieving information from the database.
