dc.description.abstractThe management and follow-up of chronic illnesses in Uganda has over time been challenging for both patients and doctors. Many patients have ended up having challenges of missing appointments, incurring high costs in routine hospital visits, lots of frustration over lengthy waits to access medical attention among many other adverse effects of the current system. For the doctors it has increasingly become difficult to keep track of appointments and patient’s health during follow-up. With the above identified challenges, a research was carried out to in order to develop a chronic illness patient management and follow-up system that would solve the existing challenges faced by patients and doctors. The research involved the following set of objectives; To review related literature and conduct a field study to identify requirements for the chronic illness patient management system. A field study regarding chronic illness patient management and follow up and was mainly carried out within health centers in Uganda. Methods used to obtain data included observation, interviews and questionnaires. Other literature was obtained through document analysis of already existing documents relating to the subject matter. To design the chronic illness patient management system. We used the structured modelling approach to design aspects of the system. Under this we developed context diagram, dataflow diagram and Enhanced Entity Relationship Diagram (EERD). To implement the chronic illness patient management and follow-up system. The system interfaces of the chronic illness patient management and follow up system were developed through use of various web development tools including HTML, JQuery, CSS among the many. To test and validate the chronic illness patient management system. Testing plan document and system validation forms were used at this stage with a major aim of identifying system errors and the level of system satisfaction and usability respectively. Identified errors were as well corrected at this stage.en_US
